Vasyli Howard Dananberg (VHD) Insoles are designed for patients with 1st ray dysfunction (1st metatarsal and great toe) that leads to hallux limitus. The Vasyli Howard Dananberg Orthotic allows the user to "tune" the function of the 1st ray by modification of proximal and distal plugs (removable wedges) to raise or lower the 1st ray. Latex free.

The Vasyli Howard Dananberg Orthotics are indicated for stage 1-3 hallux limitus. For stage 4 hallux limitus (hallux rigidus), we recommend the use of a turf toe plate. The difference between the Vasyli Howard Dananberg Orthotic and the turf toe plate boils down to flexibility vs. rigidity – the Vasyli Howard Dananberg Orthotic is used to increase the range of motion of the great toe joint, while the turf toe plate acts as a rigid splint to the joint, inhibiting motion. Question:

I am totally confused as to how to use this insole.  It came with no directions for use?  Please help!!!

Answer:

There is a great video on YouTube to help you with the placement of the tabs and which ones to use for your individual condition.  Another solution  would be to make an appointment with your doctor to make sure you get the correct placement that he requires.

Question:

Can the Vasyli Howard be trimmed?

Answer:

Yes they can.  If you turn them over you will see at the top of the insole the guides to follow for trimming the tip of toes. Go slow and trim a little at a time

Question:

I have sesamoiditis. Which of the pads should I remove, P or D, or both?

Answer:

The best approach would be to maintain the proximal (closest to) and remove the distal (farther from). Be sure to double check the placement by putting the orthotic on the ground, stand on the orthotic and visually mark the location of the sesamoid (site of the pain on the bottom of the great toe joint). The sesamoid should sit in the space created by the removed distal pad.

Question:

Hi. I have been diagnosed with functional hallux limitus. I have been looking at insoles on your website and believe that either the Hallux Trainer Insoles or the Vasyli Dananberg Insoles would be of help to me. Which of those products do you recommend? Or would you recommend something else? The insole that I think would be most helpful would provide a limited flexion during the toe off phase of walking or running, cushioning under the first metatarsophylangeal joint and mild arch support.

I’d pose two questions:

 

  1. What is the primary use of the insert?  Day to day or for sports?

  2. Truly functional, or is there a bump on the top of the great toe joint?  That’d make it structural.  Staging would be helpful – is it stage 1,2,3 or 4?  Please refer to our article on hallux limitus for more information on staging. 

 

The Vasily Dannenbergs would be great for functional hallux limitus.  The Vasily Dannenberg is actually used to create a pocket sub 1 to increase the range of motion of the great toe joint.  But that can only be used in stages 1&2 which are both functional stages.

 

If we’re a late stage 2 up to stage 4, I’d go with either the turf toe plate, spring plate or the hallux trainers.  Each one has some pros and cons regarding what type of shoes they can fit into, so the determining factor for choosing one product over the other becomes the intended use.
